How do the rating for rpgs work?

I ask since I plan on making plenty of rpg's, and most likely I'll get a dexdrive so others can play them. But....are the ratings made up by the reviewer? The maker?

Or do they get rated by the ESRB?.....

We've already selected the ratings you could give games as: Everyone, Teen, Mature, and Adult. We then have the creator choose one that best fits his or her game.

I have a Mature title coming up, and are there any important restruictions I should know about considering M rated titles?
