So, what happens when I install to the HDD? Does it do anything to my PS2? Does it install my game or RPG Maker 3?

It basically just speeds up the loading times. Nothing more than that.

Yup, you lose maybe a second or two of loading time each time it loads something. This comes in most handy when looking through character/monster models, as it essentially gets rid of load times with those, save a "loading" icon for a brief split-second.
